Managed Care of North America, Inc., along with MCNA Insurance Company and Healthplex, Inc., has reached a class action data breach settlement allowing affected individuals to file claims for up to $2,500 in documented out-of-pocket losses. According to court notices, the claim window opened in August 2026 and closes on October 19, 2026, at 11:59 p.m. Eastern Time.
MCNA Dental Settlement Details and Payout Caps
The settlement provides up to $2,500 for documented out-of-pocket losses stemming from the 2023 data incident, drawn from a $250,000 aggregate cap. According to court filings on openclassactions.com, if valid claims exceed this total cap, payouts will be reduced pro rata. In addition to cash reimbursement for losses, class members are eligible for two years of medical data monitoring that does not require a separate claim form, though activation requires the enrollment code printed on individual notice letters.
Filing a claim is free. Claimants must submit an online form or mail it with a postmark no later than October 19, 2026. The U.S. District Court for the Southern District of Florida authorized notice to the class, but final approval has not yet been granted. A final approval hearing is scheduled for November 16, 2026, at 2:00 p.m. Eastern Time in Fort Lauderdale, Florida. Defendants MCNA and Healthplex deny all allegations of wrongdoing, stating the agreement was reached to avoid the expenses and risks of ongoing litigation.
Origins of the 2023 MCNA Dental Data Incident
MCNA administers dental and orthodontic benefits primarily for individuals enrolled in Medicaid and the Children’s Health Insurance Program (CHIP). On or about March 7, 2023, the defendants discovered that an unauthorized party had potentially accessed network systems between February 26 and March 7, 2023. Notice letters to potentially affected individuals were mailed starting May 26, 2023.
The compromised data varied by individual and included names, physical and email addresses, dates of birth, Social Security numbers, driver’s licenses, government-issued IDs, health insurance plan details, Medicaid and Medicare ID numbers, and treatment histories. While court documents classify the event as potential unauthorized access without attributing it to a specific threat actor, contemporaneous 2023 reports linked the breach to the LockBit ransomware operation, which claimed responsibility publicly. That assertion came directly from the attackers rather than formal judicial findings.
Class Member Requirements and Deadlines
The settlement class includes all living individuals in the United States who received a notice stating their private information was potentially compromised during the incident. To file a claim, class members must use the Class Member ID printed on their notice letter to log into the online portal. Third-party documentation is mandatory to substantiate any financial losses claimed for reimbursement.
No payments have been issued yet, and no official distribution date has been announced by the court. Class members who wish to opt out of the settlement or file an objection must also do so by the October 19, 2026 deadline. Holders of notice letters are advised to retain their documents securely, as the printed enrollment code remains necessary to activate the included medical data monitoring services.
